Can't Afford the Filing and/or Service Fees Required in Your Action?

For any fees assessed by the court and/or service fees assessed by the Waukesha County Sheriff's Department, parties may complete the form and addendum below and present it at the time of filing to have their request for waiver of fees reviewed by a court official.  The court official will make a determination as to whether or not some or all of the fees will be waived, deferred, or required to be paid.  The Court Officials of the Waukesha County Circuit Court require that page 2 of the form be completed in addition to the addendum even though the instructions on the form may not require it.
