Subpoena & Certificate of Appearance

This packet is used when an individual will be appearing in court and:

• needs to have someone testify on his/her behalf that is not willing to appear voluntarily, or
• there are documents to which he/she does not have access and needs to have someone who does have access present them to the court on his/her behlaf.

Note: There are specific fees that must be paid to the witness and service must also be done appropropriately for the witness to testify and or present the requeted documents.
