Our Centralized Records Team is responsible for the management and release of records within Health and Human Services. We work to ensure your client records and health information remain secure and are made available to you in a timely manner. We also respond to public records requests for Health and Human Services.
We will require you to fill out and sign an Authorization for Use & Disclosure of Health or Confidential Information form when:
If you request that your mental health, substance use disorder, HIV testing, or other human/social services records be released, you must check those specific boxes on the form.
The authorization form is not valid unless filled out completely.
The completed forms can be mailed, faxed, emailed, or delivered to the following locations:
For Human Services, Child Protective Services, Adult Protective Services, Public Health, and Outpatient Mental Health/Substance Use Records: Human Services Center |
For Mental Health Inpatient, Day Treatment or Community Support Health Information: Mental Health Center |
To ensure the security of your client records and health information, a photo ID is required when picking up records at our locations above. You may designate someone on the authorization to pick up records on your behalf, but that indvidiaul must provide a photo ID as well.
If you do not have a photo ID, please let us know ahead of time and we will work with you to ensure we can verify your identity.
Requests to inspect or recieve copies of public records for health and human services may be submitted at our Human Services Center above.
