Wisconsin Immunization Law


The Waukesha County Public Health Division provides information to assist schools in meeting the requirements of the Wisconsin Student Immunization Law (Chapter 252, Wis. Stats. and HFS 144 Wis. Admin Code). The information supplements the directions found in the Wisconsin School Immunization Requirements booklet posted on the Wisconsin Department of Health and Family Services, Division of Public Health website:

Required Reporting to Waukesha County Health Division:

  • School Report to Local Health Department 2018-2019 (form F-04002)

    Please submit this form to the Waukesha County Health Division by the 40th school day which is on or around October 31st, 2018. Fax to:

The Public Health Division will be closed to the public for 2018 holiday observances on the following dates:

  • THANKSGIVING DAY and THE DAY AFTER THANKSGIVING will be observed Thursday, November 22, 2018, and Friday, November 23, 2018
  • CHRISTMAS EVE and CHRISTMAS DAY will be observed Monday, December 24, 2018, and Tuesday, December 25, 2018
  • NEW YEAR'S EVE and NEW YEAR'S DAY (2018-2019) will be observed Monday, December 31, 2018 and Tuesday, January 1, 2019
