Waukesha County Community Health Collective

Established in 2026

The Waukesha County Community Health Collective is a collaboration between Waukesha County Public Health/Health and Human Services and the county's four major hospital systems: 

  • Aurora Health Care
  • Ascension
  • Froedtert ThedaCare
  • ProHealth Care 

By working together to gather community health data, share insights, and coordinate efforts, the Collective helps partners learn more while duplicating less. This collaboration strengthens community health planning, identifies shared priorities, and supports informed decisions that improve the health and well-being of everyone in Waukesha County. As the Collective continues to grow, we look forward to expanding our partnerships and engaging additional organizations that share our commitment to improving community health through collaboration.
