This requires that the defendant and/or defense counsel return the Criminal Status Sheet within 21 days of receipt. For your convienece both Word and Fillable PDF versions have been posted.

This requires that the defendant and/or defense counsel return the Plea Enclosure Form within 21 days of receipt. If seeking a payment plan, you must complete and return the Financial Statement found here. The Notice of Hearing as to the Return Date is NOT A MANDATORY APPEARANCE. It is only a deadline for you to return the Plea Enclosure Form. NO APPEARANCE IS REQUIRED.
