Hours:
We are assisting customers with passport applications by appointment only.  Please contact our office to check availability at (262) 548-7010.

Passport Fees: 
Application Fees - Application fees are forwarded to the U.S. Department of State.  Only personal checks, money orders and bank drafts are accepted.

Execution Fees and Photos - Are paid separately from the application fee and paid directly to the County Clerk's office.  Cash or Credit card may be used as payment.

Note: Do not sign the application until the time of your appointment. 

Present Proof of U.S.  Citizenship:

You may use a previous U.S. Passport, a certified birth certificate or a naturalization certificate. Note: a certified birth certificate has a registrar's raised, embossed, impressed or multicolored seal, registrar's signature, and the date the certificate was filed with the registrar's office, which must be within one year of your birth.

Present Proof of Identity:

You may use a previous U.S. Passport, a naturalization certificate, a certificate of citizenship a current and valid drivers license, government ID or military ID.

One Passport Photo:
(Available in County Clerks office)

  • You cannot wear glasses.
    • If you cannot remove your glasses for medical reasons, please include a signed note from your doctor with application.
  • Taken in clothing normally worn on a daily basis.  
  • You cannot wear a uniform, clothing that looks like a uniform, or camouflage attire. 
  • You cannot wear a hat or head covering.
    • If you wear a hat or head covering for religious purposes, submit a signed statement that verifies that the hat or head covering in your photo is part of traditional religious attire  worn continuously in public.
    • If you wear a hat or head covering for medical purposes, submit a signed doctor's statement verifying the hat or head covering in your photo is used daily for medical purposes.
    • Your full face must be visible and your hat or head covering cannot obscure your hairline or cast shadows on your face.

For all Minors Under age 16:

The minor MUST appear in person. All applications for children under 16 require both parent's or legal guardians' consent. For more on special requirements please click here.

Time:

Application time will take approximately 30 minutes.

Passport processing time: 

Routine Service Due to COVID the State Department is unable to estimate when customers will receive their passports or have their application documents returned.
