UNCLAIMED FUNDS HELD BY WAUKESHA COUNTY

Pursuant to Wisconsin Statute § 59.66, unclaimed funds are reported by municipal Treasurers and County departments. The Waukesha County Treasurer is required to post public notice of all unclaimed funds.

If you are the owner of unclaimed funds currently in the possession of the Waukesha County Treasurer, you must submit a notarized affidavit swearing to ownership of the unclaimed funds. Click here to access a blank copy of the Affidavit of Ownership and Indemnity Agreement.

Please note: If the Unclaimed Funds App is utilized, most of the information needed will autofill on the Affidavit of Ownership and Indemnity Agreement form.

Instructions for completing the affidavit:

Please fill in the case number and dollar amount as stated in the published notification, as well as your name, address and phone number.

The Affidavit of Ownership and Indemnity Agreement must be signed by the claimant/owner of the funds and notarized by a notary public. The notary public must witness the signing of the affidavit. Notary Publics are available at most banks or the Treasurer’s office has one on staff who would be glad to assist you.

Please note: a copy of the claimant’s driver’s license or other picture I.D. must be included for identification. If you are claiming funds on behalf of a business, a copy of your business card must also be included.

Once we receive the affidavit and identification, it will take approximately 30 days for you to receive a check. We will mail the check to the address provided on the Affidavit of Ownership and Indemnity Agreement. The check will be made payable to the same person or entity the original check was payable to.
