🔙 DMV REAL ID REQUIREMENTS
• Current passport or certified birth certificate
The birth certificate must be on CERTIFIED PAPER WITH RAISED SEALS.
All Register of Deeds in the State of Wisconsin can issue any birth certificates
from any county in Wisconsin from October 1, 1907 to the present.
• Social Security Card (Can be obtained at the Social Security Office)
• For married couples:
if you are married and changed your name, you will have to show your Marriage
Certificate. This also will have to be on CERTIFIED PAPER WITH RAISED SEALS.
If you were married in Waukesha County, you can get your Marriage Certificate here.
All Register of Deeds in the State of Wisconsin can issue any Marriage Certificate from
any county from June 21, 2015 to the present.
