 Vehicle Modification Program of ADRC of Waukesha County

Eligibility and Application Process

Up to $7,000 in funding to help individuals with electric/power wheelchairs or scooters gain greater independence by modifying their personal vehicle for easier transportation in the form of a lift device for their vehicle to transport an electric/power wheelchair or scooter.

Steps to apply for funding for the vehicle modification program:
  1. Consult with a contracted mobility provider to determine the best modification for your vehicle and to ensure your vehicle qualifies for the modificationComplete an ADRC Vehicle Modification Program application and return to ADRC
    A quote from the mobility provider is sent to ADRC where it is reviewed and processed
    • A list of those contracted with the ADRC will be provided
  2. Complete an ADRC Vehicle Modification Program application 2. and return to ADRC.
  3. A quote from the mobility provider is sent to ADRC where it is reviewed and processed
  4. If approved, the mobility provider can complete the installation of the modification, and payment is given directly to the provider

Eligible individual must meet these requirements:
  • be a resident of Waukesha County
  • aged 18 or older, AND:
  • have a physical disability* which significantly impairs your ability to drive, ride, or access a personal vehicle; or that requires use of a mobility device or transfer aide which cannot be accommodated in a personal vehicle without modifications

Eligible vehicle:
  • vehicle owned by you, or an immediate family member 
  • mobility provider must determine your vehicle qualifies for the modification

Taxi/RideLine Program Application

The Aging and Disability Resource Center of Waukesha County (ADRC) sponsors subsidized transportation services for ambulatory, non/limited-driving older adults 60 years of age and older, and for individuals with disabilities age 18-59. Users of the transportation services are required to complete an eligibility application. When approved for transportation services, applicants receive a discounted fare.

The Shared-Fare Taxi Program is available to Waukesha County residents who are non/limited-drivers and are 60 years of age or older, or under 60 years of age with a disability designation. Individuals must be able to enter or exit a car with little or no assistance.

Individuals must complete an Application and be certified before using the program. If the taxi program does not serve the area in which a trip is to be made, an individual may be eligible for RideLine Service and should complete the full application. For ambulatory individuals, the RideLine Application will also provide Taxi program access.

RATES

Rates vary by taxi provider. All eligible ride costs will be reduced by $7.00, paid by the ADRC.

Reservations must be made at least 24 hours in advance. Allow additional time to assure preferred date and time of the request. Each taxi provider has their own schedule and preferences when it comes to scheduling. Please contact them directly for more information.

The RideLine Program is available to Waukesha County residents who are non/limited-drivers and 60 years of age or older, or age 18-59 with a disability designation who are non-ambulatory and using a wheelchair or scooter. Individuals must complete an Application, which includes a Fare Determination Form and a Disability Designation Form, and be certified by the ADRC before using the program.

Service provided for trips within Waukesha County. Out of County trips must be pre-approved and may be available to parts of Milwaukee County for medical services ONLY if service is not duplicated in Waukesha County.

Fares will be determined by the RideLine Fare Determination Form completed at the time of application.  Fares for individuals will be determined based on the person's income.

No Saturday, Sunday or Holiday service.

Fares begin at:
$4.75: one-way trip within same community;

$6.00: one-way trip, origin and destination is from one community to another;

$9.20: one-way trip, out of county when preauthorized for medical appointments.

Maximum fares will be paid by persons who choose not to divulge financial data on the Fare Determination Form.

 

Shuttle Service is available to Waukesha County residents who are 60 years of age or older and residing in the community sponsoring the service. Service is curb to curb and individuals must be ambulatory. There is no application or certification required to use the shuttle programs.
