• The Public Health Immunization clinic is open by appointment only.  If you would like to schedule an appointment, please call 262-896-8430, option #5.    

• Please bring all written immunization records, if available.
  

 Immunizations Provided & Fees:

• Cash, charge, or check accepted
• Medicaid/Medicare will be billed for covered immunizations. Please check with your insurance provider for coverage questions.    
• Vaccines Offered:

WIC Clinic is open for services by appointment only. For more information call 262-896-8440. Enter thorugh the Public Health main entrance.
