 Petition for Appointment of Guardian – Form No.:  JN-1501

 Uniform Child Custody Jurisdiction and Enforcement Act Affidavit – Form No.:  GF-150

 Nomination of Guardian by Parent or Child 12 years of age or older – Form No.: JN-1510

 Statement by Proposed Guardian – Form No.: JN-1514

 Proposed Letters of Guardianship and Dispositional Order Appointing Guardian – Form No.:  JN-1530

GENERAL INFORMATION

You must file a separate Petition for 'Emergency' Guardianship if seeking more than one form of Guardianship.

After you have obtained Service on all interested parties, you must complete and file with the Court:

 Affidavit of Service (Chapter 48 and 938) - Form No.: JD-1825

Per Wisconsin Statutes, a guardian ad litem will be appointed for the minor at the time of case filing.
