Online Jury Qualification Questionnaire

The Juror Qualification Questionnaire is the first step in the process of selecting jurors to serve on trials. It is not a summons to appear for jury duty, but only a questionnaire as required by Ch. 756, Wis. Statutes to determine your eligibility to serve as a juror.

Throughout the calendar year, as jury trials are scheduled, jurors will be sent a Summons to serve on jury duty. Summons are mailed to randomly-selected jurors approximately six weeks prior to jury service on the date(s) of service as shown on the summons.
