Excusal from Jury Service

You must submit your questionnaire before any excusal can be considered.

Requests for medical excusal should include a written statement from a treating physician indiciating the condition you suffer from and the prognosis for recovery. 

Any other requests to be excused from jury service can only be granted by the Jury Judge.

Requests to be excused from jury service must be submitting to the Jury Coordinator at least 10 days prior to scheduled jury service and should include a daytime phone number. Requests may be faxed to the Jury Coordinator at 262-896-8228, or mailed to:

Jurors will receive notification by mail of the status of their request.

Your Juror ID Number should be included on all correspondence.
