Passport Fees: Personal checks, money
orders, and bank drafts accepted for both application fees and execution fees,
cash accepted for execution fee only.

Note: you pay the application fee to the
"U.S. Department of State" and the execution fee to the
"Waukesha County Clerk"

To apply for a U.S. Passport you Must:
|
|
1. Provide Application Form DS-11
|
Application forms are available in the County Clerk's office or
to download an
application form, please click here Note: Do not sign the application
until the Clerk instructs you to do so.

2. Present Proof of U.S. Citizenship
|
You may use a previous U.S. Passport, a certified birth
certificate or a naturalization certificate. Note: a certified birth
certificate has a registrar's raised, embossed, impressed or multicolored
seal, registrar's signature, and the date the certificate was filed with the
registrar's office, which must be within one year of your birth.

3. Present Proof of Identity
|
You may use a previous U.S. Passport, a naturalization
certificate, a certificate of citizenship a current and valid drivers license,
government ID or military ID.
|
|
4. Two Passport Photos
(Available in County Clerks office)
|
The photographs must be 2x2 inches in size, identical, taken
within the past 6 months, color or black and white, full face, front view with
a white or off-white background. The picture should be taken in normal attire,
no uniforms or hats. If you normally wear glasses, a hearing device, wig or
similar articles they should be worn on your picture. Dark glasses or
nonprescription glasses with tinted lenses are not acceptable.

|
The minor MUST appear in person. All applications for
children under 16 require both parent's or legal guardians' consent. For
