| Present Proof of U.S. Citizenship: |
You may use a previous U.S. Passport, a certified birth certificate or a naturalization certificate. Note: a certified birth certificate has a registrar's raised, embossed, impressed or multicolored seal, registrar's signature, and the date the certificate was filed with the registrar's office, which must be within one year of your birth. |
| Present Proof of Identity: |
You may use a previous U.S. Passport, a naturalization certificate, a certificate of citizenship a current and valid drivers license, government ID or military ID. |
One Passport Photo:
(Available in County Clerks office) |
The photograph must be 2x2 inches in size, taken within the past 6 months, color, full face, front view with a white or off-white background. The picture should be taken in normal attire, no uniforms or hats. If you normally wear glasses, a hearing device, wig or similar articles they should be worn on your picture. Dark glasses or nonprescription glasses with tinted lenses are not acceptable. |
